If you are enrolled in retiree or COBRA coverage for any part of the year, THT will automatically upload your 1095-C form to your member portal. To retrieve the form, visit log into the THT Member Portal and navigate to the Documents section. Open the document and "Print" or "Save" the document.
If you were employed for any part of the year, your employer will provide you with your 1095-C. You will need to contact your employer directly to request a copy or for assistance.
