When submitting an eligibility appeal, it is important to provide enough information for Teachers Health Trust (THT) to fully understand the circumstances surrounding the request. A strong appeal should clearly explain what happened, why the request was not submitted within the required timeframe (if applicable), and why the member is requesting an exception or reconsideration. Members should also include any documents that support their explanation. THT reviews each appeal individually and considers the information and documentation submitted by the member. Providing complete and accurate documentation helps ensure a thorough review.
Members should provide:
A detailed explanation of the situation.
The reason the request could not be completed within the required timeframe.
The date the qualifying life event occurred.
The date the member became aware of the issue, if applicable.
Any circumstances that contributed to the delay.
Contact information in case additional information is needed.
Additional Supporting Documents May Include:
Court Orders
Legal Documentation
Written Statements
Insurance Carrier Correspondence
Employer Communications
Medical or emergency-related documentation when relevant
Any other records that help explain the circumstances
Deadlines and Timing
Appeals should be submitted as soon as possible after the member becomes aware of the issue. A determination letter will be mailed to the member within 2–4 weeks of submitting the appeal.
Delays in providing supporting documentation may extend processing times.
Additional documentation may be requested during the review process.
